Our concierge plan. If you have chronic ongoing health care issues, this is the plan for you.
We will work with your primary care to manage chronic health conditions.

- Initial Intake and 8 follow up visits with Mary Page
- 8 Health Coaching visits with either Becky or Cindy
- Access to all Advanced testing
- Unlimited Messaging
- 20% off supplements
- 20% Discount on Neurofeedback services
$215/Month X 12 months
NEUROFEEDBACK FM ADD-ON PLAN
INCLUDES:
- Initial FM Consultation (90-120min) $250. This includes an in-depth history, and recommendations for testing and supplements.
- All testing is done through independent labs, or Lab Corp. You can frequently use insurance to cover a portion of the cost. All labs are done at my cost, with no mark up.
- Follow up visits are billed at $75/30min visit. We will review all lab work and give you recommendations for supplements.
- All Supplements are provided at a 20% discount from retail price.
SINGLE COMPREHENSIVE FUNCTIONAL MEDICINE EVALUATION
INCLUDES:
- Initial FM Consultation (90-120 min.) $450. This includes an in-depth history, and recommendations for testing and supplements.
- All testing is done through independent labs, or Lab Corp. You can frequently use insurance to cover a portion of the cost. All labs are done at my cost, with no mark-up.
- This plan includes a follow up consultation to go over any lab work that was conducted at the initial visit.
- Follow up visits are billed at $75/30min visit.
- All Supplements are provided at a 10% discount from retail price.
